Prevention in Primary Health Care Program 2010-2012

WentWest is currently involved with the Prevention in Primary Health Care (PIPHC) Program in collaboration with NSW Health and the Heart Foundation.

The primary objective of the program is to build capacity in General Practice settings to provide support for patients who have been identified with a lifestyle risk factor for chronic disease by referring them to a lifestyle risk factor management (LRFM) service.

Program benefits include:

  • A better understanding of the practice’s particular referral needs, including:
    • which patients to focus on (who has greatest need; who gets most benefit)
    • what enablers and barriers exist to referrals
    • identification of new referral services and limitations of existing services;
  • An improved practice processes for referral;
  • An improved practice processes for tracking referral outcomes;
  • A better understanding of patient reaction to referrals.

 

The program focus is on:

  • SNAPO risk factors – Smoking, Poor nutrition, excessive Alcohol, Physical inactivity and Obesity; and
  • Referral of patients to life style related risk factor management services.

Five General Practices are currently participating in this program. After the completion of the program the results, resources and strategies will be available to General Practices in the WentWest area.

Education Integration Project - 3rd Cluster Meeting

The 3rd Cluster Meetings will begin in May. Cluster Meetings form the basis of the Education Integration Project conducted by WentWest in partnership with UWS and USyd.

Your participation is critical to the success of the vertically integrated education in your area and will enhance your clinical knowledge, promote collegiality between learners at all levels, improve teaching efficiency, create lateral thinking and generate enthusiasm.

These are open to Medical Students, Registrars, PGPPP and GP Supervisors. Details are as follows:
