SHAPE Diabetes Support Service (DSS)

What is the SHAPE Diabetes Support Service (DSS)?

SHAPE Diabetes Support Service (DSS) is a branch of the SHAPE (Sessions for Health and Physical Exercise) program specifically for people diagnosed with Type 2 diabetes.

The aim of this program is to:

  • Educate participants on diabetic appropriate diet and nutritional guidelines;
  • Encourage a more active lifestyle; and
  • Prevent the onset of complications associated with poorly controlled diabetes

What is included in SHAPE DSS?

SHAPE DSS offers participants access to:

  • An initial assessment with an allied health professional to plan an appropriate health and fitness program;
  • 8 healthy lifestyle group sessions combining diet, nutrition and exercise education with a practical exercise component;
  • Take home resources.

 

Who should participate in SHAPE DSS?

SHAPE DSS is available to western Sydney residents who either live or see a GP or GP Registrar in the WentWest geographical area.

To be eligible and to refer into SHAPE DSS you need:

  • To be diagnosed with Type 2 diabetes and hold a current Medicare card;
  • A GP care plan and referral form for "Group allied health services under Medicare for patients with type 2 diabetes”. Once the GP care plan and referral form are completed by your doctor they can fax it to the WentWest confidential fax line on (02) 8208 9941.

Education Integration Project - 3rd Cluster Meeting

The 3rd Cluster Meetings will begin in May. Cluster Meetings form the basis of the Education Integration Project conducted by WentWest in partnership with UWS and USyd.

Your participation is critical to the success of the vertically integrated education in your area and will enhance your clinical knowledge, promote collegiality between learners at all levels, improve teaching efficiency, create lateral thinking and generate enthusiasm.
