Australian Better Health Initiative (ABHI)

What is ABHI?

Australian Better Health Initiative is a national program from the Council of Australian Governments to address the concern of increasing chronic illness. The program was announced in February 2006 with a focus on promoting good health and reducing the burden of chronic disease that represents almost 80 per cent of the total burden of disease in Australia.

ABHI in the WentWest Region

WentWest is implementing an ABHI program, to integrate Hospitals, GPs, and Community health to ensure continuity of care for patients. WentWest is working in partnership with Sydney West Area Health Service to integrate the hospital services with GPs and improve communication to ensure a smooth transition for patients moving from the hospital to the community.

WentWest is also working with the Community Health Centre, to achieve collaboration between GPs, community nurses, allied health professionals, and other care providers who manage and support the chronic patient to keep them healthy in the community. Both projects are being piloted in Blacktown.

How does ABHI aid a General Practitioner (GP)?

Integration means more support for the GP as the GP, hospital and community health centre work together to achieve the chronic disease management for each patient. The following are examples of how integration can aid GPs in providing patient care:

  1. Continuing care coordinators in the hospital will coordinate services for hospital patients, and information about admissions and treatments will be sent directly to the patient's GP.
  2. The care coordinators in the hospital will facilitate case conferencing during the patient's stay to ensure appropriate communication between the hospital and the GP regarding the patient's treatment.
  3. Community Nurses will support GPs to manage their patients with chronic conditions. They may monitor patients based on the care plan, between reviews and coordinate services such as rehabilitation programs, domestic assistance etc, to assist patients in remaining healthy in the community based on the GPs advice.
  4. GPs will receive support from the GP liaison nurse to conduct case conferencing and to coordinate communication between community health and the hospital.
