Disease Specific Clinics
Setting up a clinic in your practice could improve patient health outcomes through regular structured health care as well as benefiting the practice through optimising the use of your resources and time.
Benefits to Patients:
- Patients can obtain additional support and strategies to self-manage their health;
- Chronic disease patients can receive preventative, structured and regular ongoing care.

To set up a clinic, there are only three essential steps involved:
Step 1: Set up your room and equipment
Step 2: Choose the Model of Care for your clinic
Step 3: Set up and start the administration of the clinic.

The 3rd Cluster Meetings will begin in May. Cluster Meetings form the basis of the Education Integration Project conducted by WentWest in partnership with UWS and USyd.
