Home Medicine Review (HMR)

Also known as Domiciliary Medication Management Review (DMMR)

The HMR is a consumer-focused, structured and collaborative health care service provided in the community setting to optimise consumer understanding and quality use of medicines. It involves the consumer, their general practitioner, their pharmacy and other relevant members of the health care team.

An HMR will provide the GP with comprehensive, up-to-date information about the medicines, complementary products, over-the-counter medicines, devices and other prescriptions being used by the patient.

The patient will receive assistance with medications, aids and devices to improve compliance, and early detection and management of medicine-related problems.

This service can be offered to any person who is living at home, by their regular GP, who is at risk of medication misadventure and can be done once every 12 months or sooner if there has been a significant change in the patient's condition or medication requirements.

Risk factors known to predispose people to medication-related problems include:

  • Currently taking five or more regular medications;
  • Taking more than 12 doses of medication per day;
  • Significant changes made to the medication regimen in the last three months;
  • Medication with a narrow therapeutic index or medications requiring therapeutic monitoring;
  • Symptoms suggestive of an adverse drug reaction;
  • Sub-therapeutic response to treatment with medicines;
  • Suspected non-compliance or inability to manage medication related therapeutic devices;
  • Patients having difficulty managing their own medicines because of literacy or language difficulties, dexterity problems or impaired sight, confusion/dementia or other cognitive difficulties;
  • Patients attending a number of different doctors, both general practitioners and specialists;
  • Recent discharge from a facility/hospital (in the last four weeks).

Become an Accredited Pharmacist for HMR

Candidates for accreditation must be a pharmacist registered in Australia, fulfill preparatory (stage 1) training requirements and meet qualifying experience relating to medication management reviews (by providing evidence of Medication management review-related experience and competencies). Accreditation can be obtained through the Australian Association of Consultant Pharmacy (AACP). Re-accreditation is required every three years.

Become an Approved Pharmacy for HMR

The Pharmacy must be Quality Care Pharmacy Program (QCPP) accredited (or intend to pursue accreditation within two years) before they can apply to Medicare Australia to become a Home Medicines Review service provider. Applications available through link below.
