GP Medical Educator

This is a great opportunity to join our talented education team, responsible for developing and implementing high quality, innovative general practice training for GP registrars in western Sydney. This role will be key to the development and support of the Supervisors mentoring the Registrars within the training program. Training is based on an apprenticeship model in hospital and community settings with extensive use of the training opportunities available in private general practices.
We are seeking GPs who are available 2 days (4 sessions) and who have the following skills and qualifications:
· Qualified GP with FRACGP or equivalent
· Experience or an interest in clinical teaching/tutoring
· The ability to work in a team and mentor GP registrars
· Creativity, problem solving and negotiation skills
· Great communication skills
· Computer literacy
· Educational experience preferable
· Tertiary qualifications in education preferable

Area Services Coordinator
We are currently recruiting for an Area Services Coordinator to join a dynamic, fun and innovative team. The role involves working with local general practice and other primary health care stakeholders, to plan and deliver a comprehensive range of quality primary health care services for the benefit of the local community. Key responsibilities include:
- Establish and build relationships with general practice, general practitioners and practice staff through regular practice visits
- Provide support and advice through the provision of training, information and resources
- Develop, obtain or adapt resource materials for the general practice support program
- Establish and develop links with other primary health care providers e.g allied health professionals and hospitals
- Train, mentor, and support other team members in your project focus area
- Contribute to the development of appropriate primary health care program time lines, implementation and evaluation
- Conduct needs analysis of key stakeholders to assess knowledge, problems and concerns regarding the primary health care program
This full time role requires someone with:
- Degree qualifications in health, social science or business
- Ability to develop and build professional relationships
- Ability to facilitate training programs and an understanding of adult learning principles
- Good verbal and oral communication
- Demonstrated program management skills
- Demonstrated initiative and ability to work independently
- Knowledge and experience of working in a change environment
- Confident, proactive, flexible, professional
- Demonstrated problem solving skills
- Intermediate level computer literacy and proficiency with Microsoft Office Suite

Integration Project Officer
WentWest is recruiting for an Integration Project Officer to join our team to effectively implement and support integration and clinical coordination projects to facilitate continuity of care and service for patients in Western Sydney, as well as support for integration of general practice and primary health care services.
Key responsibilities include:
- Conduct project management and support activities for WentWest’s integration and clinical coordination programs including HealthOne, Antenatal Shared Care, Coordinated Care and Supplementary Services, and Connecting Care/Severe Chronic Disease Management Program
- Develop appropriate program timelines, implementation, and evaluation tools and documents to support the above programs
- Plan and support stakeholder engagement including primary care providers, Community Health and Hospitals
- Develop project materials and conduct implementation activities for WentWest’s primary health care integration initiatives
- Undertake needs analysis such as surveys of relevant health professionals and consumers/patients to assess knowledge, problems and concerns regarding the clinical coordination and integration programs
- Prepare quality business communication documents including reports, flow charts, spreadsheets, minutes, surveys, plans, general correspondence and others as required
- Develop and document relevant policies and procedures
- Promote relevant training, funding and networking opportunities to service providers
- Prepare recommendations and reports as necessary to meet reporting requirements
- Contribute to the organisation-wide development and maintenance of an up-to-date service and contact directory to support both patients and providers
- Conduct research and data collection for the integration and clinical coordination programs
- Work collaboratively with clinical, integration, practice support, Close the Gap, health promotion and other WentWest teams to facilitate sharing of knowledge and resources and development of project materials
This full time role requires someone with:
- Relevant tertiary qualifications and/or experience for example in health, business or social science
- Project management skills, including the development and implementation of project plans and the proactive monitoring and documenting of project progress against deliverables
- Effective planning, organisational and problem solving skills
- Excellent interpersonal skills and positive attitude, open-mindedness and ability to work in a changing environment
- Customer relation skills, including the ability to develop and maintain professional relationships/networking
- Ability to prioritise workload and meet strict deadlines whilst providing project support
- Administrative support skills, including the ability to utilise Microsoft Excel, Access and electronic databases where required, as well as the ability to prepare quality business documentation including project plans, needs assessments, meeting minutes, stakeholder surveys and others as required
- Demonstrated experience or skills in conducting research (e.g. surveys, written reports, needs assessments) to support relevant projects
- Excellent written and verbal communication skills
- Attention to detail and superior organisational skills
- Demonstrated initiative and ability to work independently and as part of a team
- Knowledge of or experience working within a primary health care system
- Excellent computer skills and experience using Microsoft Office Suite

Local Community Partnerships Coordinator, The Hills Area, Western Sydney
As one of the first Tranche of Medicare Locals (ML) in Australia, WentWest, the leader in integrated primary health care, is looking for a Local Community Partnerships Coordinator (LCP) Coordinator's for the Hills area of Western Sydney to:
· Forge and facilitate an effective Local Community Partnership in the Hills area
· Connect care for communities in Hills area by working collaboratively with health care consumers, clinicians and community service providers to reduce fragmentation and make the health system easier for patients, carers and health care professionals to navigate
· Drive the development and implementation of innovative, evidence-based primary health care integration strategies for the Western Sydney Medicare Local by working with and supporting clinicians and stakeholders in the Hills area
To be successful in this role you will need tertiary qualifications in either health, social science or business and 3 years experience working in the health care sector in driving projects and engaging effectively with internal team members and external stakeholders such as GPs, community health staff and hospital administrators. In addition you will need experience in:
· coordination of complex programs, including the ability to deliver innovative health program outcomes
· planning, developing and delivering models of care for identified patient cohorts
· supporting and engaging clinical and non-clinical care providers to adopt systems of improved shared care
· developing high level written documentation such as strategic documents, briefing papers and plans
· dealing with multiple stakeholders including at a Senior level
