What is it? A questionnaire for asking all practice staff about the registrar’s behaviour. It is made using a series of statements, to which respondents say how much they agree or disagree. There is also space for written text feedback. The form can be done anonymously. Who developed it? This was developed for use in the Northumbria VTS in the UK by the North Northumberland trainers group. How is it done? The form is distributed to staff members, who answer and give their forms back to the supervisor or the registrar. What does the supervisor do? The supervisor feeds back to the registrar what they did well and what they need to improve and may suggest or support ways of doing this. Again, the tool should be used as a starting point for a discussion, rather than just the result itself.
