What is it? This questionnaire asks practice staff to score the behaviour of the registrar in the following areas: · Problem solving, making a diagnosis, management plans · Emergency care · Working within limits of competence · Attitude to and relationship with patients · Working with colleagues · Probity · Health issues that may affect practice Who developed it? This tool is available on the London Deanery Formative Assessment toolbox in the UK How is it done? The form is given out to practice staff members who rate the registrar according to the scale given on the form. Note that the results are just ratings, as there is no space for free comment. What does the supervisor do? The supervisor discusses the ratings with the registrar when they are returned. The discussion may wish to focus around the reasons for given ratings, as there is no space for free comment on the form. Practice tips This form may be useful in formalizing an impression that a registrar needs work in one of these areas.
